Observation

When licensor arrived the front door/primary entrance and exit, was answered and licensor was asked to wait a minute. When door was closed licensor could hear the dead bolt being latched. When talking to staff and provider this licensor learned that not only is teh dead bolt latched but there is also a chain latch above the dead bolt that is sometimes used. Licensor observed that the door does not latch properly and opens on its own if the dead bolt is not used.
